Description:

The Physical Therapist will partner with Early Learning programs (serving children birth- age 5) in St. Joseph County to coordinate the needs of early learning programs, provide training and education on delays, disabilities and challenging behaviors to teachers, families, and the community.

The Physical Therapist provides evaluations and treatment in various settings, assessing the gross motor area of development. This therapist will provide treatment plans and outcomes reflected in each client’s authorization in accordance with the payor requirements, using therapeutic modalities and procedures to maintain and increase skills. The Physical Therapist will also communicate with staff and families regarding results of the evaluation, treatment and progress, provide parent training, and make suggestions for home programming.

At times, additional First Steps services and supports will be required to sustain progress for these children. A First Steps Physical Therapist is responsible for implementing therapeutic support in the home and/or the child’s natural environment. The therapist will plan, implement, evaluate, and document each therapeutic plan to ensure proper programming in conjunction with the child’s parent/guardian during home visits.

REQUIREMENTS & QUALIFICATIONS Essential Functions:

Assist in evaluating persons served with other therapists or individually.

Communicate the results of the assessment/screening as directed.

Determine each client’s adaptive equipment/orthopedic equipment and/or assistive technology needs.

Therapy:

Collaborate with family and provide individual therapy when appropriate to maintain and improve gross motor development; muscle strength, ranges of motion, positioning, and expanding mobility options.

Provide and/or obtain necessary equipment for proper positioning and play if applicable.

Communication:

Communicate effectively with other staff, agencies, and community programs, persons served and their parents/guardians.

Records And Reports:

Keep daily progress notes, make reports on client progress in accordance with authorization limits or reporting timelines required by payor.

Complete discharge reports and/or exit skills summaries for clients who have met their goals or are transitioning to the school system, or another provider.

Participate in case conferences, staff meetings, and parent group meetings when requested.

Write annual meetings to determine service plans, attend workshops and conferences to further professional development.

General Responsibilities:

Be responsible for the health and safety of the people served.

Advocate to protect the rights of persons served.

Participate in training as required by supervisor or for all LOGAN staff.

Plan instructional activities for clients and assist families in accessing community activities.

Respect confidentiality of persons served and cultural diversity, in accordance with LOGAN policies.

Qualifications:

Education:

Registered and licensed Physical Therapist in the State of Indiana.

Experience:

0-2 years’ experience in the field (Level 1) or 2+ years’ experience (Level 2).

Prior experience working with pediatric, IDD, DD, and/or ASD populations is preferred but not required.

Knowledge, Skills, And Abilities:

Communicate respectfully and appropriately with staff, supervisors, and agency personnel.

Work collaboratively with other therapeutic disciplines.

Establish and maintain effective relationships with persons served and family members/guardians.

Be creative in exploring new ideas.

Work independently and use sound judgment.

Currently registered/license with Health Professions Bureau of Indiana.

Indiana only: Have a valid driver’s license and reliable transportation; be able to drive to visit sites.

Enroll in state of Indiana First Steps provider.

Able to lift 50 pounds throughout a session.
