Overview Our Home Care Assistants are vital, providing aid in participants’ homes and contributing to care planning as a member of our interdisciplinary team (IDT). Reporting to the Home Care Supervisor, the Home Care Assistant will provide safe delivery of personal care services to WelbeHealth participants in their home, as scheduled and directed.

Day-to-Day Responsibilities
Provide hands-on personal care and assistance with ADLs to participants in their homes
Assist participants with Instrumental ADLs (laundry, light housekeeping, shopping, light meal prep)
Measure and document vital signs as instructed
Act as a medical escort on vehicles to medical appointments
Assist the Rehabilitation team by carrying out home exercise programs as indicated
Provide translation services as needed
Job Requirements
High school diploma or equivalency
Current Basic Life Support (BLS)/First Aid Certification required
Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred
Bilingual preferred
Must be registered with Home Care Registry prior to start date
