The role is located in

and emphasizes the integration of psychosocial/physiologic assessments, patient education, and collaborative decision‑making to meet the complex needs of patients and families throughout the continuum of care. Responsibilities
Clinical Practice and Care Management: Perform assessments and formulate individualized care plans based on patient data and trends. Patient Education and Discharge Planning: Adapt educational programs to individual needs and incorporate patient and family teaching strategies. Advocacy and Conflict Resolution: Advocate for patient rights, identify potential conflicts, and seek counsel for resolution. Quality Improvement: Participate in unit‑based initiatives and utilize evidence‑based practice to enhance outcomes. Leadership and Professional Development: Serve as a clinical resource, preceptor, and mentor, and maintain continuing education and competencies. Qualifications
One‑year clinical care nursing experience. Graduate of an accredited school of nursing. Current Louisiana State license as RN and BLS certification. Proficient in verbal and written communication and computer skills. Seniority Level
