Registered Nurse – Hospital in the Home (HiTH) Adult Central Intake Service
Responsibilities
Demonstrates autonomy in practice by performing a thorough and ongoing holistic nursing assessment, including admission assessment of patient and patient support systems; identifies, prioritizes, and documents current and potential problems; develops an individualized plan of care addressing needs and concerns.
Collaborates with patient, support systems, and healthcare team to meet patient’s present, discharge, and outcome needs; implements plan of care within 24 hours of admission; initiates interdisciplinary referrals and revises plan regularly.
Provides individualized, ongoing teaching to patient and support systems throughout the continuum of care, using evidence‑based practice.
Communicates mutually respectfully with the interdisciplinary team, uses active listening and assertive communication, advocates for patient rights, and fosters inclusive team support.
Performs all treatments and procedures in accordance with evidence‑based policies, procedures, and patient care standards, collaborating with MD and team members.
Accurately documents patient information efficiently and timely; utilizes nursing process on required forms to ensure effective communication.
Provides all aspects of medication process: assessment, administration, monitoring, evaluation of patient response; teaches patient and support systems safe medication use and initiates referrals to pharmacy.
Actively participates in Quality Performance and Improvement process, identifies problems, collects and analyzes data, implements corrective actions, and sustains improvements; maintains culture of patient safety.
Delegates appropriate care tasks to LPN, CNA, and ancillary personnel according to scope of practice; supervises and provides direction; mentors and fosters development of coworkers from novice to expert.
Requirements
Education:
Graduate of an accredited school of nursing; currently licensed in the State of New York or permittee. Membership in a professional organization suggested.
Experience:
Basic knowledge of nursing theory and practice; knowledge of appropriate technology and practice standards for area of responsibility; sound clinical skills; knowledge of hospital structure, objectives, and standards of practice; positive communication skills.
License:
New York State Registered Nurse license required.
