Key Responsibilities
Perform nursing assessments, ensuring clinical competencies are met.
Evaluate client/patient physical conditions and address service needs.
Coordinate care plans based on collaboration with treatment teams.
Provide health education and engage clients in disease prevention strategies.
Administer and oversee medication requirements, ensuring compliance.
Monitor vital health metrics and consult with medical providers as needed.
Complete necessary documentation in accordance with company policies.
Advocate for and arrange medical appointments for clients/patients as required.
Education, Experience, and/or Credential Qualifications
Current Registered Nurse licensed in the state of practice.
Must be willing to obtain licensure in additional states.
Comfortable providing telehealth services.
FQHC: BCLS certification required; ACLS certification preferred.
Additional Qualifications
Successful completion of a background check, including criminal record and driving record.
Current driver’s license, acceptable driving record, and current auto insurance.
Completion of New Hire Orientation and annual training requirements, including Relias training.
Physical Requirements
Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects.
Sedentary work involves sitting most of the time but may involve walking or standing for brief periods.
Requires repetitive movements of hands, fingers, and arms for typing and writing during work shifts.
