Family Nurse Practitioner (FNP)

This is a full-time, outpatient-focused role within a supportive, multidisciplinary clinical environment. The Family Nurse Practitioner will deliver comprehensive primary care services to patients of all ages, with a strong emphasis on preventive care, wellness, and management of acute and chronic conditions.

Essential Position Responsibilities
Provide comprehensive primary care services, including preventive care, acute illness management, and chronic disease treatment for patients of all ages
Perform physical examinations; order, interpret, and evaluate diagnostic tests
Develop and manage individualized treatment plans
Educate and counsel patients on disease prevention, health maintenance, and lifestyle modification
Collaborate closely with physicians, specialists, and clinical staff to ensure coordinated, high‑quality patient care
Maintain accurate, timely documentation in the electronic health record (EHR)
Participate in care coordination, quality improvement initiatives, and population health efforts
Qualifications
Education:
Graduate of an accredited Nurse Practitioner program (MSN or DNP)
Licensure & Certification:
Active or eligible New York State Family Nurse Practitioner (FNP) license; national certification required
Experience:
Prior primary care or family medicine experience preferred; new graduates are welcome to apply
