We are currently seeking an experienced Registered Nurse (RN) for a Travel Contract Hospice Home Health job in Milwaukee, Wisconsin. Schedule: Day shift (schedule details to be discussed)
Setting: Hospice / Home Health
Responsibilities
Provide skilled nursing care to hospice patients in their homes.
Perform patient assessments, symptom management, and care planning.
Administer medications and treatments per physician orders.
Educate patients and families on disease progression, comfort measures, and care plans.
Coordinate care with physicians, hospice team members, and caregivers.
Maintain accurate and timely clinical documentation.
Qualifications
Active RN license (Wisconsin or compact accepted).
Hospice and/or Home Health nursing experience preferred.
BLS certification required.
Strong communication, time management, and organizational skills.
Ability to work independently in a home-based environment.
