You’ll collaborate with the interdisciplinary team to ensure holistic care during end-of-life journeys.**Key Responsibilities:*** Conduct spiritual assessments and develop individualized care plans* Provide spiritual counseling and support directly or through coordination with community clergy* Participate in interdisciplinary team meetings to address spiritual needs* Maintain accurate documentation of spiritual care services* Offer education and support to hospice staff on spiritual care* Perform memorial or funeral services as appropriate* Participate in bereavement activities and provide ongoing support* Serve as liaison to community clergy and spiritual counselors**Qualifications:*** Graduate of accredited seminary or school of theology (BD, MDiv, or equivalent)* Minimum 1 year of hospice or palliative care experience* Previous pastoral ministry or chaplaincy experience* Comfort working in an interfaith setting and respecting diverse beliefs* Current BLS certification* Valid California driver’s license, reliable transportation, and proof of insurance* Strong communication and interpersonal skills* Knowledge of hospice philosophy and end-of-life spiritual care**About Seaport Hospice**Seaport Hospice is part of the **Pennant Group**, a nationwide network with over 300 locations.
