The Senior Social Worker - HealthCare for Homeless Veterans (HCHV) Coordinator is assigned to the Wilmington VA Medical Center and is a member of the homeless care team who provides services to homeless veterans throughout Delaware and collaborates with members of the homeless care team who provide other homeless services such as supportive housing case management, transitional living, and Veterans justice outreach as part of other VA Homeless programs. Qualifications
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy English Language Proficiency in basic written and spoken English Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to

to verify that the social work degree meets the accreditation standards for a masters of social work Licensure: Licensed or certified by a state to independently practice social work at the advanced practice level which includes an advanced generalist or clinical examination (unless grandfathered by the state). Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post‑masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty Demonstrated Knowledge, Skills, and Abilities: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area and role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Driving Requirements: The work is primarily community‑based. The incumbent must operate a government vehicle to visit Veterans in the community and periodically transport Veterans in the Government vehicle. Incumbent must pass all VHA requirements to operate a government vehicle prior to transporting Veterans. Preferred Experience: Experience in Mental Health, substance abuse and community outreach. Serve as the Medical Center's primary point of contact for homeless outreach services Develop and maintain service delivery systems for homeless veterans, including collaborating with other agencies, building and maintaining an effective treatment team Ensure veterans are eligible for VA services Act as the Primary HCHV Contract Liaison and monitor contract compliance as needed May serve as the Contracting Officer's Representative (COR) – be a backup COR or work in collaboration with a COR Maintain required COR training every two years Collaborate with primary COR to maintain budget awareness and contract spending knowledge throughout the performance period for each contract Coordinate with the VA and community agencies, organizations and groups to learn of current services, resources and benefits available for homeless veterans Participate as a member of Housing Alliance CoC Collect and submit veteran information as outlined by VA Homeless Operations Management and Evaluation System (HOMES) formats and procedures Work with hospital's multidisciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to homeless veterans Review the records of admissions and discharge dates of veterans utilizing the VA‑contracted emergency shelters Ensure accuracy of bills submitted by the contracted emergency shelters and also certify bills Monitor the homeless hotline and ensure that all calls are responded to within 24 hours for urgent calls and 72 hours for route calls Conduct psychosocial assessments to determine needs of veterans and family members Participate in discharge planning with VA colleagues to ensure homeless veterans on the medical floor are provided with a safe‑living arrangement for discharge Develop therapeutic relationships with Veterans to help remove psychosocial barriers and establish plans to achieve and sustain permanent housing Provide ongoing therapeutic intervention with the patient and significant other to assist them in dealing and coping with the psychosocial issues of mental health, aging, illness and disability, dealing with loss and grief Assist veterans with transportation needs to attend VA and other social service–related appointments via bus tickets, GSA vehicle, etc. Assist Veterans who encounter problems obtaining services from the VA and other service delivery agencies by providing patient advocacy and linkage to resources Provide consultation/education to veterans and families on community resources, advance directives and VA benefits Act as a consultant to other team/staff members Act as liaison between VA and community resources – including marketing and public relation duties Address advance directives and organ donation in accordance with the standards of the work area Provide wellness/prevention education and facilitate patient/family support groups as appropriate Participate in committee assignments – as deemed necessary by the service manager Maintain data and statistical compilations to comply with JCAHO, CARF, VA and medical center policy and procedures Selected applicants may qualify for credit toward annual leave accrual based on prior work experience or military service experience.
