Summary The Senior Social Worker – HealthCare for Homeless Veterans (HCHV) Coordinator is assigned to the Wilmington VA Medical Center and is a member of the homeless care team who provides services to homeless veterans throughout Delaware. The coordinator collaborates with team members who provide supportive housing case management, transitional living, and Veterans justice outreach as part of other VA Homeless programs.

Duties

Serve as the Medical Center’s primary point of contact for homeless outreach services.

Develop and maintain service delivery systems for homeless veterans, including collaboration with other agencies and building a multidisciplinary treatment team.

Ensure veterans are eligible for VA services.

Act as the Primary HCHV Contract Liaison and monitor contract compliance, serving as COR, backup COR, or collaborating with a COR as required; maintain required COR training every 2 years.

Collaborate with primary COR to maintain budget awareness and contract spending knowledge throughout the performance period for each contract.

Coordinate with VA and community agencies, organizations, and groups to learn of current services, resources, and/or benefits available for homeless veterans; participate as a member of Housing Alliance CoC.

Collect and submit veteran information as outlined by VA Homeless Operations Management and Evaluation System (HOMES) formats and procedures.

Work with hospital’s multidisciplinary teams and community agencies to develop more effective ways of delivering healthcare services to homeless veterans.

Review admissions and discharge records of veterans using VA‑contracted emergency shelters, ensure accuracy of bills submitted by the contracted shelters, and certify bills.

Monitor the homeless hotline and ensure all calls are responded to within 24 hours for urgent calls and 72 hours for routine calls.

Conduct psychosocial assessments to determine needs of veterans and family members.

Participate in discharge planning with VA colleagues to ensure homeless veterans on the medical floor are provided with a safe, living arrangement for discharge.

Develop therapeutic relationships with Veterans to remove psychosocial barriers and establish plans to achieve and sustain permanent housing.

Provide ongoing therapeutic intervention with the patient and significant other to assist them in dealing with mental health, aging, illness, disability, loss, and grief.

Assist veterans with transportation needs to attend VA and other social service‑related appointments via bus tickets, GSA Vehicle, etc.

Advocate for veterans encountering problems obtaining services from the VA and other agencies, linking them to resources.

Provide consultation/education to veterans and families on community resources, advance directives, and VA benefits.

Act as a consultant to other team/staff members and liaison between VA and community resources, including marketing and public relation duties.

Address Advance Directives and Organ Donation in accordance with the standards of the work area.

Provide wellness/prevention education and facilitate patient/family support groups as appropriate.

Participate in committee assignments as deemed necessary by the service manager.

Maintain data and statistical compilations to comply with JCAHO, CARF, VA, and medical center policy and procedures.

Qualifications Basic Requirements

United States Citizenship (non‑citizens may be appointed only when it is not possible to recruit qualified citizens).

English Language Proficiency (must be proficient in basic written and spoken English).

Education: Master’s degree in social work from a CSWE‑accredited school of social work.

Licensure: Licensed or certified by a state to independently practice social work at the advanced practice level.

Grade Determinations : GS‑12. In addition to the basic requirements, the following qualification criteria must be met for each grade.

Experience/Education.

Minimum two years of experience post advanced practice clinical licensure, with one year at the GS‑11 level or above.

Licensure/Certification.

Must be licensed or certified at the advanced practice level and provide supervision for licensure.

Demonstrated Knowledge, Skills, and Abilities.

Skill in specialized interventions, ability to incorporate complex causes in dosing and treatment, knowledge of measuring service effectiveness, and ability to provide specialized consultation and write policies and procedures relevant to the service area.

Preferred Experience.

Experience in mental health, substance abuse, and community outreach.

Driving Requirements: Must operate a government vehicle to visit Veterans in the community and periodically transport Veterans. Must pass all VHA requirements to operate a government vehicle prior to transporting Veterans.

Education Foreign education must be accredited by a U.S. Department of Education recognized institution and a Certificate of Foreign Equivalency must be provided with the transcript.
