Job Description Metro is seeking a motivated and passionate Physical Therapist (Per Diem) to provide in-home care. All levels of experience are encouraged to apply; we will train the right new graduate. Job Type: Per Diem Pay: $65–$85 per visit Location: In and around Middle Island Schedule: Flexible (see patients any 7 days of the week) What We Offer

Accept and treat patients within your designated geographic area. Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans. Develop a plan of care and treatment goals in consultation with the physician, patient, and family. Plan, prepare, and implement customized therapy programs to restore function and promote independence. Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques. Provide timely and accurate documentation for each visit. Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning. Deliver care in compliance with agency policies and professional practice standards. Qualifications
Physical Therapist Program Graduate. Licensed Physical Therapist in New York or eligible. Mandatory certifications: First Aid, AED, and CPR. Home‑health experience preferred but not required; a mentor will be provided for documentation and compliance. Ability to travel to patients’ homes within the assigned geographic territory.
