Licensed Vocational Nurse Home Health (San Luis Obispo County) 45-50 per hr
Omega Home Health Care Services, Inc. is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care for homebound patients in Los Angeles County, Kern County, Santa Maria, Lompoc, San Luis Obispo, and surrounding areas.
Pay Range $45.00 per daily visit – $50.00 per daily visit (commensurate with experience)
Responsibilities
Provides skilled nursing care to clients as directed by the registered nurse and physician's plan of care in accordance with agency policies.
Promotes and maintains client health through teaching and appropriate rehabilitative measures under the guidance of the registered nurse who is assigned to the same case. Utilizes agency educational material as appropriate.
Assists clients in learning appropriate self‑care techniques.
Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals.
Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy.
Assists the physician and registered nurse in performing specialized procedures for which competency has been established.
Prepares equipment and materials for treatments observing aseptic technique as required.
Keeps abreast of changes and trends affecting nursing care and licensed nurse functions.
Attends client care conferences, communicates regularly with the registered nurse.
Communicates current health status and needs of clients to appropriate health professionals.
Schedule
Flexible scheduling: schedule visits with patients.
Minimum availability: at least two days per week, one weekend day per month, two holidays per year (one winter, one summer).
Ability to accommodate changing two available days each week for candidates with additional jobs.
Qualifications
LVN License (Required)
Driver's License (Required)
Active BLS/ACLS/PALS certification
Reliable transportation and proof of auto insurance
Self‑directed, able to work independently with little supervision while meeting performance metrics
Physical, mental strength and flexible work hours
Ability to remain calm under pressure
Proficiency with electronic health records and web‑based applications
At least 1 year of Home Health experience
Work Location
Multiple locations within San Luis Obispo County and surrounding areas
No remote work opportunities
