Base Pay Range:

$69,000–$70,000 per year

Position Summary Provides supportive psychosocial services to patients and families. Supports pre‑death bereavement services, addresses grief, behavioral needs and risk assessments for complicated grief, and applies principles of palliative care and hospice philosophy. Offers direct patient/family counseling, referrals to community resources, and helps develop coping strategies. Works within a hospice team to provide holistic care.

Duties & Responsibilities

Assess psychological, social, grief, and environmental needs of patients and families, initiating contact within 72 hours of admission.

Provide social work services—including psychosocial assessment, counseling, consultation, education, resource referral, advocacy, and discharge planning—to empower patients and families.

Participate in interdisciplinary care plan development and weekly team conferences.

Conduct initial post‑death bereavement assessment and coordinate care with the Bereavement Counselor.

Organize and attend educational forums, seminars, and training on psychosocial/bereavement issues for hospice staff, volunteers, health professionals and the public.

Contribute to team meetings and ongoing care plan updates, including bereavement plans.

Document all social work interactions, keeping records of time, mileage, and other required documentation.

Provide bereavement consultation and support as assigned.

Support cost‑containing practices while maintaining hospice care quality.

Follow safety practices and Hawaii Care Choices policies and procedures.

Educate patients, caregivers, volunteers, co‑workers and the community; participate in meetings, committee work and special projects.

Skills & Knowledge Requirements

Independent sound judgment in planning and providing psychosocial care.

Strong assessment and critical thinking skills.

Ability to deliver unsupervised psychosocial services in private residences.

Self‑motivation and ability to work independently or collaboratively with the interdisciplinary team.

Sensitivity to the needs of the dying and interest in hospice care.

Valid Hawai‘i State Social Worker license.

Education & Experience Requirements

Graduate of a Council of Social Work Education–accredited school with a master’s degree in social work.

Hospice experience and LCSW provider status preferred.

Proficiency with private vehicle, computer, telephone, pager, and personal cellular phone (no driving while mobile).

Hours Exempt position requires flexibility; may need to work after‑hours, weekends, and holidays as needed.

Pay $69,000–$70,000 annually (base salary; actual offer depends on experience, education, geography and other factors).
