Job Summary The Hospice Palliative Social Worker is responsible for development, planning, implementation and evaluation of psycho/social interventions for patients followed in the Home Health, Hospice IP/OP, and Palliative programs. This role focuses on identifying patients with complex psycho/social needs, assisting with eligibility determination for social programs and funding sources, and coordinating community assistance from various special funds and agencies. The Social Worker operates as the mental health professional of the team, providing crisis intervention and constructing care plans. Services may be delivered in person, by phone, or via telehealth.

Responsibilities

Provides Home Health, Hospice IP/OP and Palliative Social Work across outpatient and inpatient settings.

Assesses patients and families for psychosocial risk factors based on referrals or case findings.

Completes psychosocial assessments and delivers interventions.

Actively participates in Interdisciplinary Team (IDT) meetings, contributing to patient care plans.

Intervenes with patients and families regarding emotional, social, and financial consequences of illness.

Provides supportive counseling for patients/families dealing with illness, catastrophic diagnosis, changes in living situations and bereavement.

Intervenes with patients experiencing complicated grief and explores bereavement related to possible death.

Provides anticipatory guidance throughout the continuum of care.

Provides non‑pharmacological pain and symptom management, reinforcing patient and family personhood.

Assists Hospice Chaplain in developing care plans.

Advocates for patient and family empowerment and independence in health‑care decision‑making.

Facilitates family meetings and collaborates in discharge planning activities.

Provides clinical guidance and consultation to primary teams, ensuring continuity of care.

Supports families with complex dynamics affecting patient care and discharge.

Educates patients, families and interdisciplinary team on post‑acute options and choices.

Participates in complex rounds, discharge planning huddles, case conferences and whiteboard rounds.

Intervenes in cases of child abuse/neglect, domestic violence, guardianship, foster care, mental health placement, adult/elderly abuse and sexual assault.

Supervises and mentors Social Workers enrolled in Master level programs.

Participates in on‑call rotation as required by the department.

Qualifications

Master’s degree in Social Work required.

Current license from the State of New Mexico as a Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW) or Licensed Master Social Worker (LMSW).

Hospital social work or discharge planning experience in a relevant setting.

Working knowledge or experience in case management.

National Case Management certification or SW Palliative Care Certification; ACHP‑SW or APHSW‑C preferred.

Compensation Base pay range $30.00/hr – $51.10/hr.
