Responsibilities
Monitor and manage patients on anti‑coagulation therapy
Perform patient assessments and provide education on medication usage and safety
Coordinate care with healthcare providers and ensure optimal treatment outcomes
Maintain accurate documentation of patient progress and therapy adjustments
Skills Needed
Strong clinical assessment and communication skills
Ability to educate and support patients effectively
Attention to detail and excellent organizational skills
