Liberty Healthcare Corporation is accepting applications for a new Clinical Case Manager position with the California Conditional Release Program (CONREP). This program exclusively serves adult sex offenders who are living in communities throughout California – specifically, those individuals designated as sexually violent predators (SVPs).
Program Objectives
Maintain community safety by preventing client recidivism.
Transition clients from hospital treatment into the community.
Secure community‑based housing.
Proactively detect and address client forensic risk factors.
Provide behavioral health services, case management, 24/7 monitoring, and client transport.
Specific Responsibilities
Track client progress on community reintegration goals (e.g., employment, budgeting, life support funds, driver’s license).
Identify and address barriers to successful client reintegration in the community.
Secure community‑based services and supports for clients.
Coordinate a wide range of client discharge planning activities.
Track, document, and report on client developments and program processes.
Ensure timely completion and quality of client assessments, treatment plans, and other services.
Assist in the oversight of client evaluations at the Coalinga State Hospital (infrequent travel).
Provide crisis management, assessment, and treatment services to clients as necessary.
Collaborate with community providers, social service agencies, Department of State Hospital representatives, and other stakeholders.
Provide backup coverage for the Clinical Director and participate in the on‑call rotation.
Benefits
Starting salary between $90,000 and $105,000 (depending on qualifications).

Qualifications
Professional licensure (or eligibility for licensure) in California as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).
Assessment, counseling, and case management experience.
Experience identifying and securing community‑based supports and services.
Experience providing services to justice‑involved individuals (especially adult sex offenders or other forensic clients) is preferred.
Residency in California.
