Locum Tenens - Nurse Practitioner
Position Type: 1099 independent contractor Specialty: Geriatric Medicine Location: Mishawaka, IN Rate: Open & Negotiable ($90 - $100 hourly) Shift: Varied (will be discussed in interview) Start Date: As soon as credentialed. End Date: 6 months from Start date.
Position Details: Patient Population: Geriatric. Setting: Nursing home care. Responsibilities: to provide primary medical management in collaboration with the Physician. Required Procedures: • Performs assessments in collaboration with the physician and as needed. • Develops and implements appropriate plans of care to PO participants. • Evaluates participant s physical complaints and provides appropriate treatment. • Provides participants and caregiver teaching and education. • Functions as a member of the Interdisciplinary Team (IDT). • Provides daily oversight of PACE Organizations Clinic. • Perform rounds in the POs contracted nursing homes. • Regularly informs the IDT of the medical condition of each participant, remaining alert to pertinent input from other team members, participant s caregivers, as well as documenting changes in a participant s medical record consistent with documentation policies.
EMR System: NA Support Staff: Yes On-Call/Weekends: Yes Submission Requirements: Active IN License - REQUIRED. BLS, CDS, DEA REQUIRED. Board Certified by ANCC or AANP REQUIRED. A minimum of one (1) year working with the frail or elderly population strongly preferred Join us today!
