The Admissions Nurse will play a key role in welcoming new residents, coordinating admissions, and ensuring a smooth transition into our facility. Responsibilities
Conduct initial assessments of new residents to determine care needs and develop individualized care plans.
Coordinate with families, physicians, and other healthcare providers to gather necessary medical information and ensure seamless admissions.
Educate residents and families about the facility’s services, policies, and care processes.
Document resident information accurately in compliance with state and federal regulations.
Collaborate with the interdisciplinary team to ensure continuity of care.
Provide emotional support to residents and families during the transition process.
Participate in quality improvement initiatives to enhance the admissions process.
Qualifications
Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in [State].
Minimum of [1-2] years of nursing experience, preferably in long-term care or geriatrics.
Strong assessment and documentation skills.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
