Baptist Medical Center Jacksonville is looking for a Licensed Clinical Social Worker (LCSW) to join our growing, highly skilled team in the Palliative Care department. This is a PRN day shift opportunity working Tuesday, Wednesday and a third day that is negotiable along with a commitment of 15 Saturdays per year. This LCSW will work in a fast-paced environment where they will assist patients and families with serious or complex illnesses in planning to determine goals of care. The LCSW is a high performing clinician practicing independently as a lead member of the palliative care interdisciplinary team.
Responsibilities
Assess psychosocial needs of patients and family members to provide services that enhance patient functioning, emotional well‑being, and continued access to resources in the community.
Provide clinical consultation and update care teams.
Facilitate family meetings focusing on supportive counseling, guidance, education, advocacy, and coordination of hospital and community‑based social services.
Assess coping, support systems, and provide appropriate interventions and resources to support patients and their families.
Demonstrate flexibility, proactive problem‑solving, and excellent communication skills.
Qualifications
Florida LCSW in good standing with Florida’s Department of Health (registered interns will not be considered).
ACHP‑SW certification preferred.
At least 2 years of palliative care experience.
At least 2 years of social work experience.
Master’s degree required; doctorate preferred.
