About the Role We are seeking an experienced Home Health Registered Nurse for a full‑time, 13‑week assignment based out of Grand Rapids, Michigan. This role offers consistent daytime hours and the opportunity to provide high‑quality care across several established coverage areas.

Orientation – Branch location: Grand Rapids, MI
Requirements
Home Health experience required
Proficiency with HCHB (Homecare Homebase)
OASIS experience required
Strong clinical assessment skills
Ability to work independently in the field
Reliable transportation
Responsibilities
Provide skilled nursing care to patients in their homes
Complete OASIS assessments and documentation accurately and on time
Develop and implement individualized care plans
Collaborate with interdisciplinary team members
Educate patients and families on care plans, medications, and safety
Maintain a high standard of professionalism and patient-centered care
