Overview The Client Coordinator is primarily responsible for recruitment, hiring, staffing and scheduling of caregivers that meet the client’s needs. The role collaborates with operations and executive leadership in performance management of field caregivers when necessary. The position requires flexibility and teamwork with the health care team, providing ongoing supervision, education, support, and evaluation to field caregivers.
Compensation USD 20–22 per hour plus monthly incentive opportunity.

Essential Job Functions
Coordinate care for assigned clients, supporting goal‑directed care plans from admission to discharge.
Perform on‑site supervisory visits to conduct and document client contact by phone or visit at least every six months.
Develop and implement a written plan of care for home‑care clients based on on‑site visits.
Review and reassess the client’s plan of care during on‑site visits and update the plan with necessary changes every 12 months or whenever significant changes occur.
Maintain current awareness of the abilities and limitations of each field caregiver.
Demonstrate thorough knowledge and adherence to company policies and procedures.
Actively engage in cost‑containment initiatives and effectively assign staff to cases.
Identify and solve problems; use initiative and good judgment to reach quality decisions.
Evaluate staff performance through field visits, documentation review, client records, and personnel files.
Maintain excellent rapport with clients and field caregivers, promoting harmonious relationships.
Assist in development and implementation of regular in‑service training for all staff to improve client care and safety practices.
Provide orientation and training for new home‑care staff members.
Coordinate staffing needs according to branch location guidelines.
Promote home‑care services in the community.
Manage and maintain staffing schedule per branch location guidelines.
Schedule employee shifts and manage back‑fill activities for tardiness or absences.
Serve as primary point of contact with client families regarding staffing schedules.
Assist with rotating on‑call duties for weekday and weekend activity with appropriate staff as needed.
Complete home‑care intakes after regular office hours, when necessary.
Provide detailed information of agency operations and procedures to clients during intake/initial contact.
Participate in location recruiting activities for all staff, including identifying, scheduling, and interviewing potential candidates.
Act as supervisor of direct care services.
Requirements
High school diploma or GED.
Must be able to work after‑hours on‑call schedule.
Proficient Microsoft Office skills.
Proficient typing skills.
NAC/HCA Certification.
Valid driver’s license and insurance.
Preferences
Six (6) months previous agency staffing or recruiting experience.
One (1) year previous office work experience a plus.
Healthcare experience a plus.
Bilingual a plus.
Other Skills/Abilities
Adhere to confidentiality standards and professional boundaries at all times.
Knowledge and understanding of compliance with regulations.
Comfortable working with families with limited resources.
Remain calm and professional in stressful situations.
Ability to multi‑task.

Physical Requirements
Must be able to speak, write, read, and understand English.

Prolonged sitting, walking, standing, bending, kneeling, reaching, twisting.
Must be able to sit and climb stairs.
Must have visual and hearing acuity.
Must be able to respond physically and mentally to emergency situations in the home or during transport.
Environment Performs duties in an office environment with occasional field visits during agency operating hours. Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
