8‑hour shifts Full‑time position Responsibilities
Identify the medically related social service needs of residents and ensure those needs are met by the appropriate discipline. Maintain contact with family (with resident’s permission) to report changes in behavior, psychosocial functioning, and discharge planning, and encourage family participation in care planning. Assist residents and responsible parties with financial and legal matters such as drafting petitions for guardianship or conservatorship, and pre‑paid funeral arrangements. Coordinate discharge planning services, including placing a resident on a waiting list for community congregate living, arranging home‑care services for residents returning home, and facilitating transfer arrangements to other facilities. Through assessment and care‑planning, identify and seek ways to support residents’ individual needs, preferences, routine practices, concerns, and choices. Qualifications
Bachelor’s degree in social work or a related human services field (sociology, special education, rehabilitation counseling, psychology). Minimum of a Limited Social Work license (LSW required; LBSW preferred). At least one year of supervised social‑work experience in a health‑care setting working directly with individuals. Excellent customer‑service skills and a commitment to delivering passionate, innovative care. Team‑player attitude with a dedication to supporting colleagues and getting the job done. Contact
