Community Integrated Case Manager Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness across Arizona.

Job Summary

The Community Integrated Case Manager provides case‑management services in shelter or housing programs and serves as the liaison between CBI Shelter and Housing Programs and the CBI PCMH clinic. The incumbent assists the patient and treatment team in planning, connecting, outreaching, advocating, coordinating, and monitoring the patient’s needs and progress while in the program.

The role supports patients in accessing health services, financial assistance, employment, education, social services, and natural supports, and helps develop independent living skills.

The Case Manager assesses patient needs, develops and monitors individualized treatment plans, and documents services accurately and timely.

The role requires a recovery‑philosophy approach and promotion of a safe recovery environment for clients.

Responsibilities

Coordinate and monitor patient care in shelter or housing programs.

Advocate, organize, and connect clients with health, financial, educational, employment, and social services.

Develop individualized treatment plans and track progress.

Document services and maintain accurate records.

Qualifications

High school diploma or GED required. Associate’s degree or higher in a field related to behavioral health preferred.

Recovery from alcohol or drugs and/or GMH for equal to or greater than one consecutive year preferred.

At least one year of experience in a behavioral health position providing outreach and engagement activities, or equivalent combination of education and experience.

Current Arizona driver’s license and a 39‑month MVR.

Completion of Behavioral Health Technician (BHT) training in accordance with A.A.C. R9‑10‑101.33 and CBI Clinical Policies and Procedures.

Peer Certification, if applicable.

Arizona Fingerprint Clearance card.

Jail clearance, if applicable.
