THE POSITION The Residential Habilitation Program Coordinator will provide services to Certified Family Home (CFH) providers and participants under the direction of the Residential Habilitation Contract Manager and Hub Supervisor. These services include training, orientation and oversight to CFH providers. Services also include the development of Program Coordination Plans, Program Implementation Plans and completion of Provider Status Reviews.
This position services our South East Idaho area, with a caseload of 60. Position primarily a work-from-home position, but candidate must reside somewhere near Boise, ID.
There will be travel requirements for face to face visits in the Boise, Nampa/Caldwell and surrounding areas.
ESSENTIAL FUNCTIONS
Provide services and training for CFH Providers, per directions of contract and governing bodies
Completing initial plans and PIP's
Provides initial & annual training to CFH providers
Conducts initial and annual face to face visits with each CFH provider and participant on caseload
Establishes and maintains participant files for all participants on caseload
Maintains on-going contact with CFH providers as needed
Contacts the CFH provider on a quarterly basis or more often, as needed
Completes required provider status reviews, annual satisfaction surveys, and data reports
Ensures participants programs are implemented and/or modified and transferred as necessary when a participant chooses to relocate to a new CFH
Meets regularly with Hub Supervisor to review delivery of services, paperwork, reporting and to participate in on-going training
Qualifications
Qualified Intellectual Disabilities Professional (QIDP)
Bachelors degree in one of the following professional categories: psychology, social work, occupational therapy, speech pathology, professional recreation or other related human services profession
Minimum of one (1) year experience working directly with persons with an intellectual disability diagnosis or other developmental disabilities
Minimum of one (1) year of experience writing habilitation programs and providing skill training
Ability to communicate effectively through oral and written communications
Ability to work interdependently and independently with a high degree of self-direction
Ability to use Microsoft Office and Google Suites
Effective time management, problem solving and organizational skills
Good public relations and social skills
Completion of employment process including verification of references
Maintain a current drivers license, vehicle insurance and good driving record
Successful completion and clearance of criminal background check
