Registered Nurse – Home Health / Hospice

Guaranteed Hours:
40/week Contract Length:
13 weeks with possible extension
Job Summary: The Registered Nurse (RN) provides skilled home health and/or hospice nursing care to patients of all ages in their residences. This role coordinates patient care with the interdisciplinary team, patients/families, and referring agencies while ensuring high-quality, compliant, and patient-centered care.
Key Responsibilities: Assess patients in stable and emergent home settings and identify physical, psychosocial, and environmental needs Develop, implement, evaluate, and document individualized Plans of Care (POC) Provide skilled nursing care including wound care, IV therapy, venipuncture, pain management, catheter care, and medication administration Educate patients and families on disease management, medications, home safety, and self-care Ensure accurate, timely documentation and compliance with payer and regulatory requirements (OASIS) Supervise and provide clinical direction to HHAs/STNAs/LVNs Coordinate referrals and serve as liaison among patients, families, physicians, and care teams Participate in interdisciplinary meetings and provide on-call services as assigned Follow OSHA and infection control standards at all times
Required Qualifications: Active State RN license 1–2 years RN clinical experience;
2+ years home health experience required Experience with
Homecare Homebase
and
OASIS CPR certification Valid driver’s license, reliable transportation, and insurance Strong assessment, communication, organization, and independent work skills
Preferred Qualifications: BSN xsgimln CHPN certification
Education: Graduate of an accredited ADN or Nursing Diploma program (required) BSN (preferred)
