Job DescriptionJob Description A Home Care Registered Nurse (RN) provides medical and personal care to patients in their homes under the direction of a physician. They perform assessments, administer medications, monitor vital signs, change dressings, and educate patients and families on home care. A key aspect of the role is communication with physicians and other healthcare professionals to ensure coordinated care.

Care Planning: Develop and implement individualized patient care plans, collaborating with physicians, other healthcare providers, and the patient/family.
Medication Administration: Administer prescribed medications, monitoring the patient's response and documenting any changes.
Wound Care: Provide wound care, including dressing changes, cleaning, and monitoring healing progress.
Vital Sign Monitoring: Regularly monitor vital signs (e.g., temperature, blood pressure, pulse, oxygen saturation) and document any significant changes.
Patient Education: Educate patients and their families on their condition, medications, and proper home care techniques.
Communication: Maintain clear and consistent communication with physicians, other healthcare professionals, and the patient/family.
Documentation: Maintain accurate and thorough documentation of patient assessments, care plans, interventions, and communication.
Coordination of Care: Collaborate with other members of the healthcare team, including therapists, social workers, and home health aides, to ensure comprehensive care.
Skills and Qualifications: Registered Nurse (RN) License: Must be a licensed RN in the state where working.
Clinical Experience: Experience in patient care, especially in a home health setting, is beneficial.
Communication Skills: Strong communication and interpersonal skills are essential for interacting with patients and families.
Critical Thinking: Ability to assess patient needs, identify problems, and develop appropriate solutions.
Organizational Skills: Ability to manage time effectively and prioritize tasks in a fast-paced environment.
Physical Abilities: Ability to perform physical tasks such as lifting, bending, and kneeling, as well as travel to patient homes.
