We are seeking a compassionate and skilled Hospice Nurse Practitioner (NP) to join our dedicated team at Aachele Home Care and Hospice. As a Hospice Nurse Practitioner, you will play a pivotal role in providing high-quality care for patients with life-limiting illnesses, offering comfort and support to patients and their families. You will collaborate with physicians, nurses, and other healthcare providers to develop and implement individualized care plans Responsibilities: Perform comprehensive patient assessments, medical history reviews, and symptom management evaluations. Develop, implement, and revise patient care plans with the interdisciplinary team. Communicate effectively with Medical Director, interdisciplinary team, patients, and families, addressing their concerns and ensuring their needs are met. Provide guidance and leadership to hospice nurses, social workers, chaplains, and other team members to ensure high-quality patient care. Monitor patient progress, manage pain and other symptoms, and adjust care plans as needed. Conduct home visits to assess patient conditions, offer emotional support, and educate families on the hospice process and care requirements. Ensure documentation is up to date and compliant with regulatory standards. Qualifications: Master of Science in Nursing (MSN) degree from an accredited Nurse Practitioner program. Current and unrestricted Nurse Practitioner license in GA Experience in hospice, palliative care, or home health settings is highly preferred. xsgimln Valid drivers license and reliable transportation for patient home visits.
