Job Details
The Disease Coordinator in the Division of Endocrinology works collaboratively with medical staff, nursing, social service and other staff to ensure that a Diabetes Education program is implemented for all pediatric patients in both the inpatient and ambulatory settings. The disease coordinator provides education to parents and patients in the ambulatory and inpatient settings in collaboration with nursing staff in these assigned areas. Staff education and community education issues related to management of children and young adults with endocrine conditions is also a component of this role. Requires knowledge of infants to adulthood growth and development. Coordinates a disease specific registry of patients and collaborates with their providers to reach best outcomes. Uses motivational interviewing skills to administer treatments and track patient compliance in a manner that improves clinical outcomes and reduces cost. Educates patients and families on their specific disease processes and provides ongoing assessment and re-evaluation of health care needs. Assists with managing patients in care transitions between visits and hospitalizations. Responsible for monitoring patient compliance with evidence based clinical guidelines for specific diseases.
