Essential Job Functions
Review, process, compound, prepare, label, and dispense medication orders and prescriptions.
Monitor drug therapy for appropriateness, contraindications, interactions, adverse effects, and therapeutic/toxic dosing.
Communicate drug information and medication counseling to patients and health care personnel.
Perform quality assurance through drug use evaluations and medication area inspections.
Maintain accurate documentation of patient care activities and regulatory requirements.
Supervise support personnel as required.
Special Responsibilities ZSFG Inpatient Pharmacy Positions
Review, process, and dispense physician medication orders and prescriptions including IV admixtures and chemotherapy.
Consistently monitor and follow up on unresolved medication order issues, lab orders, in‑basket messages, and properly document clinical interventions in the EHR.
Provide therapeutic drug monitoring consultation with dosing calculations.
Organize, plan, and prioritize daily inpatient pharmacy workflow within the shift.
Jail Health Positions
Act as designated Pharmacist‑in‑Charge of the facility pharmacy and ensure compliance with all state and federal laws and regulations.
Behavioral Health Services Positions \ Review, process, and dispense medication orders for mental health and substance use disorder prescriptions, including specialty packaging.
Assist clients and providers with medication insurance navigation and formulary consultations.
Provide observed dosing, perform point‑of‑care tests, and direct patient counseling for substance use disorder and mental health medications.
Laguna Honda Hospital Positions
Review, process, and dispense physician medication orders and prescriptions for geriatric residents.
Prepare pharmaceuticals for dispensing by compounding extemporaneous products.
Assist in monthly patient chart review and data collection for drug use evaluation studies.
Conduct medication area inspections and maintain a professional work environment.
Participate in medication administration observations and interdisciplinary team meetings.
Minimum Qualifications
Valid Registered Pharmacist license issued by the California State Board of Pharmacy (current license status).
Possible Special Conditions
Six months of experience (1,000 hours) in an inpatient pharmacy within the last five years.
One year (2,000 hours) of pharmacy informatics experience within the last three years.
Bilingual fluency requiring a bilingual proficiency exam.
Ability to obtain and maintain security clearance with the San Francisco Sheriff’s Department.
Six months or more of work experience in pharmacy supply chain, medication inventory procurement, and/or 340B Drug Pricing Program.
