Pharmacist (2450) – Department of Public Health

City and County of San Francisco invites a full‑time, permanent pharmacist to ensure quality patient care, appropriate drug therapy, and compliance with all applicable regulations across multiple public health settings. Overview

Positions are located throughout the Department of Public Health (DPH) including Zuckerberg San Francisco General Hospital & Trauma Center inpatient and outpatient pharmacies, purchasing pharmacy, 340B program coordinator, San Francisco County jails, community behavioral health services, Laguna Honda Hospital and Rehabilitation Center, and various clinics and health centers across the city and county. Responsibilities

Review, process, compound, prepare, label, and dispense medication orders and prescriptions. Monitor drug therapy for appropriateness, contraindications, interactions, adverse effects, and therapeutic/toxic dosing. Communicate drug information and/or medication counseling to patients and health care personnel. Perform quality assurance by conducting drug use evaluations and medication area inspections. Maintain accurate and thorough documentation of patient care activities and regulatory requirements. Supervise support personnel as required. For inpatient pharmacy roles additional duties include preparing IV admixtures, chemotherapy, therapeutic drug monitoring consultations, workflow organization, and shift hand‑offs. Jail, behavioral health, and Laguna Honda positions have specific requirements detailed in the original posting. Qualifications

Licensed Registered Pharmacist in California; license status must be “Current.” Residency, internship, or pharmacy school education. College degree/specialist certification equivalent to a PhD considered. Experience: 1–5 years of relevant experience for non‑inpatient positions; inpatient positions may require 6 months of inpatient pharmacy experience within the last five years. Special conditions for selected positions: inpatient pharmacy experience, pharmacy informatics experience, bilingual fluency, security clearance, 340B program experience, EPIC certification.
