Job Description Full‑time position. Responsible for gathering essential intake information from acute care patients and their families, assisting with discharge planning, and collaborating closely with healthcare providers, social services, and community resources to meet patient needs. Acts as a patient advocate to ensure access to available services.
Job Requirements
Associate’s Degree in Social/Human Services or related field (or equivalent education/experience).
Excellent written/oral communication, interpersonal, and telephone skills.
Ability to manage multiple, changing priorities.
Physical Standards
Constantly (≥75% time):
Walking; lifting/carrying 0–5 lbs.
Frequently (25–75% time):
Standing, sitting with back support, operating machinery, exposure to radiation.
Occasionally (5–25% time):
Reaching (overhead, forward, lateral, low); climbing; twisting; lifting/carrying 5–25 lbs.; pushing/pulling 5–15 lbs.
Rarely (
Stooping, crouching, kneeling, balancing, pushing/pulling 15–40 lbs.; exposure to chemical or infectious hazards, slippery surfaces.
Visual: close paper work, VDT use; Hearing: conversation, telephone, background noise.
