STAFF NURSE – PRIMARY CARE / Bilingual / Spanish‑Preferred / Full Time (40 hrs, Variable Rotating – Mainly Days)

Job Summary The Primary Care Registered Nurse is an integral member of the multidisciplinary team, providing comprehensive, compassionate care to a diverse, medically and socially vulnerable population. The RN supports patient and family goals for optimal health and wellness.
Principal Duties and Responsibilities
Patient Care Organization
May perform all duties as described in the Brigham and Women’s Registered Nurse Job Description to meet direct care requirements of the role.
Functions effectively and safely within the scope of practice of the Registered Nurse.
Provides culturally sensitive, patient‑centered care of highest quality to patients of all backgrounds and income levels.
Triages urgent calls and assists walk‑ins, prioritizes care based on acuity, and manages clinic flow.
Assesses patient needs, develops, implements, and re‑evaluates individualized nursing care plans.
Supports patients with lifestyle changes and uses varied modalities (in‑person, telephonic, electronic, telehealth) to engage them.
Implements coaching, education, counseling, motivational interviewing, referral, and advocacy to promote health and wellness.
Evaluates and documents disease management outcomes; works closely with clinical team and family to refine care plans.
Leverages community resources and coordinates care across the continuum, including home care, outpatient clinics, emergency/urgent care, and hospital settings.
Provides basic health teaching, family‑planning education, and care for common pediatric/adult illnesses.
Facilitates advanced care planning, assisting patients and families with future healthcare decisions and documenting goals in the EHR.

Serves as a resource to team members, supports clinical and administrative staff, and mentors support staff as needed.
Delegates safely and effectively to support staff within scope of practice.
Coordinates care across team members to match patient needs, capabilities, and qualifications.

Promotes self‑management of prevention, behavioral health, and transitional care with an emphasis on wellness.

Qualifications Education:
Graduate of an approved school of nursing with current registration in Massachusetts.
Bachelor of Science in Nursing (BSN) required for newly licensed nurses.
Language Skills:
Bilingual required; Spanish/English bilingual preferred.
Experience:
One year recent (within 3 years) ambulatory nursing care experience (Surgical/Procedural and ED excluded) required.
Recent (within 3 years) nursing experience in Primary Care required.
Telephone triage experience and proficiency required.
Skills and Abilities:
Meet all skills and abilities requirements on the BWH RN job description.
Demonstrated knowledge of both hospital‑based and community‑based care and resources.
Clinical knowledge includes basic health teaching, family planning, advanced care directives, and care for common illnesses.
