Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care?

Altus Hospice is seeking a full‑time Hospice RN Case Manager (RNCM) to join our dedicated nursing team and provide exceptional end‑of‑life care throughout the community.

Schedule:

Monday – Friday 8:00 am to 4:30 pm, with some on‑call duties assigned.

Territory:

Alice and surrounding rural area.

Responsibilities

Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.

Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services.

Coordinate care as an RN Case Manager, providing comfort and support to patients and ensuring their quality of life remains paramount.

Deliver top‑notch nursing care rooted in proven principles and techniques, easing pain and bringing solace.

Educate and empower patients and families, encouraging their active participation in personalized care plans.

Share the responsibility of call duties, ensuring we are present when needed most.

Qualifications

Current, unencumbered license as a registered nurse.

Minimum one (1) year of RN experience in a medical‑surgical/acute care setting; hospice experience is a plus.

Computer proficiency, including EMR background.

Current CPR certification (state‑required).

Valid driver’s license, vehicle insurance, and reliable transportation.

Salary $76,000 – $80,000 per year (average pay range).

The pay range listed is a general guideline; final compensation will be determined based on experience, qualifications, and the specific responsibilities of the position and will be discussed and confirmed at the conclusion of the interview process.
