Description Registered Nurse, Case Manager
Act as the case manager for assigned patients and the educator for patients’ caregivers. Utilize the nursing process and the six phases of the Alive CARE visit to provide safe, end‑of‑life care to terminally ill patient/family units. Co‑ordinate and provide hospice care for home‑care patients through development, initiation and updating of a plan of care in collaboration with patients and family. Provide direct patient care, oversee the interdisciplinary team, and teach patients and families regarding disease process, symptom management, and support during difficult times. Care is delivered in the patient’s home or any facility where the patient resides or receives care. Accountable for meeting Alive Care Standards and participating in an on‑call rotation.
Essential Duties and Responsibilities
Present a Professional Image:
Represent Alive positively, maintain a professional appearance, adhere to dress code, vehicle cleanliness, and driving etiquette.
Provide Excellent Customer Service:
Conduct visits in a professional, courteous manner, project warmth, engage in purposeful conversation, and use active listening.
Assess, Plan, Intervene and Educate:
Use critical thinking and the nursing process to assess patients and families, develop and implement an interdisciplinary hospice plan, initiate interventions, evaluate effectiveness, and educate staff, family, and caregivers.
Document at the Point of Care:
Complete accurate visit documentation, update plans of care, and ensure staff have the necessary records.
Create a Lasting Impression:
Conclude visits with encouragement, provide emergency contact numbers, supplies, and express gratitude.
Complete Post‑Visit Follow‑Up:
Follow up with team members, synchronize devices, and perform spiritual release of the visit.
Requirements Education and/or Experience:
Registered Nurse with a current nursing license (BSN preferred). Minimum one year of nursing practice or equivalent experience. Knowledge of or willingness to learn hospice care; experience in hospice, oncology, home health, or long‑term care preferred.
Certificates, Licenses, Registrations:
Current Tennessee RN license or compact license; hospice certification desirable. If driving is required: current driver’s license and automobile insurance as required by Tennessee State Law.
