$55.00/hr - $100.00/hr Pay details:
$55 per Regular Visit, $100 per Start of Care or Resumption of Care visit, $75 per Recertification or Discharge visit Benefits

Director of Nursing BRIEF JOB DESCRIPTION
The Registered Nurse (RN)/Case Manager plans, organizes, and directs home care services emphasizing health education/experience. The RN/Case Manager travels to a patient’s home to administer services dictated by the physician while helping patients maintain independence and quality of life. This role offers flexibility for field staff. Full, part-time, and PRN positions are available in the Porter County area. ESSENTIAL JOB FUNCTIONS / RESPONSIBILITIES
Patient Care: Assess and chart observations of the patient’s condition and needs at each home visit Complete evaluation tasks, including medication reviews and vital signs Initiate the Plan of Care and revise as patient status changes Provide education to patients and family on home health procedures and strategies Develop a care plan with goals based on nursing diagnosis and therapeutic, preventive, and rehabilitative actions Administer medications and treatments as prescribed by the physician Provide health care instructions per assessment and plan of care Identify discharge planning needs and prepare patient and family for discharge Coordinate patient care as Case Manager when assigned; supervise home health aides as needed Coordinate with physicians, physical therapists, occupational therapists, MSWs and other team members; perform supervisory visits with CNAs as part of the care plan (every two weeks) Communication: Prepare clinical notes and update the primary physician as needed Communicate with physicians regarding patient needs and changes in condition; obtain physician orders as required Coordinate care with community health-related persons Report updates, concerns and issues to home office POSITION & QUALIFICATIONS
Home healthcare experience preferred Graduate of an accredited school of nursing Current Indiana licensure and CPR certification Supervising home health aides experience optional; not required Strong observation, communication, problem-solving, basic math, and nursing skills Ability to lift/move patients and equipment; adequate mobility and vision/hearing for nursing tasks Must be a licensed driver with insured automobile in good working order Additional Job Details
