Schedule:
8 hour shift Monday thru Friday
Responsibilities & Duties:
Work closely with hospital staff including discharge planners, physicians, social workers, case managers, inpatient units and administration to provide direct and indirect care to clients.
Work with hospital social worker to begin the housing process if needed.
Develop and maintain new and current relationships with assigned hospital(s).
Provide initial and ongoing diagnostic assessments of both hospital and agency clients in the hospital and for the agency and complete the Hospital ISP.
Provide behavioral health counseling and CPST for clients through home, office and hospital visits.
Continue to work with clients for up to three months upon discharge to ensure proper and complete linkage with resources and continuity of care.
Attend treatment team meetings.
Qualifications:
LISW/LPCC or higher required.
Previous experience in social work or counseling.
Strong documentation skills and ability to use electronic health record system.
Compassionate and caring demeanor.
Ability to build rapport with clients.
Strong leadership qualities.
Excellent written and verbal communication skills.
Valid driver’s license & insurance.
Pass background check and drug test.
