Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services throughout Arizona for individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
Job Summary The Community Bridges, Inc. (CBI) Client Stabilization Specialist is a shelter or temporary lodging site‑based position. The primary role is to work with individuals and families experiencing homelessness, ensuring they are stabilized upon entering shelter. Responsibilities include conducting HMIS intakes, providing education on rules, resources, and overall orientation, performing face‑to‑face visits, perimeter walks, and escalating issues that impact stabilization. The Specialist communicates with leadership and Client Care Coordinators on program violations, identifies complex needs for coordination of services, and completes data entry for violations, end‑of‑shift notes, and room checks. The position also identifies the need for a housing prioritization assessment for all individuals.
Skills & Requirements
High‑school diploma or GED.
Minimum 12 months of recovery from substance use and/or mental health disorders.
1–3 years of full‑time health‑care related work experience is preferred, especially exposure to behavioral and medical crisis situations.
