Responsibilities & Qualifications
Biopsychosocial assessment, conduct initial evaluation, develop plan of care to meet patient/caregiver needs.
Collaborate with other disciplines, refer to other resources, document per care plan, and document patient/caregiver teaching.
Ensure coverage criteria, document patient response, facilitate discharge and contingency planning, long‑range planning, crisis intervention.
Advance directives, AFDC, food stamps, HMOs, housing charities, Medicaid, Medicare, medical transportation, medication assistance, prior authorization, Social Security, TANF, Agency on Aging, HCBS, Medicare waiver, VCP, adult protective services, child protective services.
Knowledge of cultural issues, NASW Code of Ethics, home health care standards, short‑term acute care inpatient medical social work.
Job Details
Pay: $1,768–$1,998 per week
Assignment length: 13 weeks
Shift: 5, 8‑hour shift 08:30–17:00
At least one year of experience required
