Salary: $1,768 – $1,998 per week.
Job Details
Assignment Length: 13‑week assignment
Shift: 5 days, 8‑hour, 08:30 – 17:00
At least one year of experience required
Responsibilities / Qualifications
Biopsychosocial assessment
Conduct initial evaluation
Develop plan of care to meet patient/caregiver needs
Collaborative care
Refer to other disciplines/resources
Document care delivery, plan, goals, patient/caregiver teaching, coverage criteria, and patient response
Facilitate discharge and contingency planning
Long‑range planning and decision making
Provide crisis intervention
Educate patient/caregiver to facilitate discharge
Advance Directives and related policies (AFDC, Food stamps, HMOs, Housing Charities, Medicaid, Medicare, Medical Transportation, Medication Assistance, Prior Authorization, Social Security, TANF, Agency on Aging, HCBS, Medicare waiver, Veterans Choice Program, Adult protective services, Child protective services)
Knowledge of cultural issues, NASW Code of Ethics/Standards of Home Health Care, and short‑term acute care inpatient medical social work
