Position Overview The
Social Services Director
is responsible for overseeing the social services program in compliance with
Ohio Department of Health (ODH)
and federal regulations. This role plays a key part in admissions, care planning, resident advocacy, and discharge planning while promoting psychosocial well-being and resident rights.
What You’ll Do
Lead and manage the Social Services Department in a skilled nursing setting
Complete psychosocial assessments upon admission and as needed
Participate in interdisciplinary care plan meetings and resident conferences
Serve as an advocate for residents and families, ensuring resident rights are upheld
Address concerns and assist with grievance resolution in a professional, empathetic manner
Coordinate discharge planning and transitions of care
Maintain accurate, timely documentation per ODH and federal requirements
Collaborate with nursing, therapy, admissions, and leadership teams
Connect residents and families with appropriate community resources
What We’re Looking For
✔ High school diploma or equivalent required
✔ Bachelor’s degree preferred but not required
✔ Experience in long-term care, skilled nursing, or healthcare preferred
✔ Knowledge of ODH regulations, resident rights, and discharge planning
✔ Strong communication, organization, and problem-solving skills
✔ Compassionate, approachable, and resident-focused mindset
✔ Ability to manage sensitive situations with professionalism and empathy
