Rogue Valley Manor is hiring a Social Services Director to join our health services department. You are responsible for the psychosocial health of all Skilled Nursing Facility residents, determining admission acceptance, resident assessments, discharge planning and providing community resources to support transitions out of the SNF, and act as the grievance officer handling investigations as they come up. You will be the community appointed resident advocate for Skilled Nursing Facility residents and are responsible for ensuring community and company-wide practices are in residents’ best interests.

Understand the importance of resident rights in healthcare as well as the community as a whole, and advocate for operations, processes, and decisions that are in residents’ best interests.
Work with residents to identify any psychosocial needs which affect their well-being, mood and behavior, develop a plan to minimize or eliminate problems, and monitor effectiveness of treatment plans.
Consult with ancillary staff (i.e. wellness, restorative, activities, etc.) in setting up socialization and individual activities that will meet the unique needs of residents.
Plan for new admissions, resident transfers and understand potential needs of incoming residents.
Participate in Resident Care Conferences and contribute pertinent psychosocial information in order to develop an interdisciplinary Service Plan.
Act as resident advocate for all residents in the Skilled Nursing Facility. Support social services requirements of other levels of care as needed.
Coordinate and report on, Ethics Committee meetings, quarterly family representative meetings and resident interviews.
Serve on the quality improvement committee and contribute process improvement ideas.
Perform functions of the job in a competent, timely and effective manner than ensures maximum productivity and a high level of resident care.
Provide counseling and socialization to long term residents to help cope with elderly depression, isolation and dementia related issues.
Assist nursing and medical team with diagnosis of psychosocial impairments and assist in the counseling and care associated with restoration or maintenance of resident’s health.
Issue Medicare beneficiary notices accurately in a timely manner.
Interact regularly with skilled nursing residents in coordinating discharge, post-discharge services and needs and other resident needs from doctors, nursing, family or otherwise.
Set up home health services for all skilled nursing discharges and serve appropriate paperwork on time.
Complete social services assessments for all residents in line with the Minimum Data Set (MDS) schedule and other assessment tools as appropriate.
Manage admissions through contact and marketing with discharge planners, family member inquiries, hospital referrals, physician offices.
Responsible for checking the Common Working file and communicating to the IDT number of eligible skilled days remaining.
Interview residents/families upon admission to obtain information concerning resident’s needs, obtain Insurance information and review and obtain signatures on all admission paperwork.
Understand the levels of care available at the community and participate in the Transfer (Resident Review) Committee for awareness of resident status changes.
Understand and be able to explain Medicare covered and non-covered services.
Perform other duties as assigned.
About you:
Minimum of 3 years’ experience in social work, geriatric care or a related field.
Bachelor’s degree in social work, gerontology or a related field is preferred.
Master’s degree in social work, psychology, geriatric care, or a related field or current Licensed Clinical Social Worker (LCSW) license issued by a state accredited program is preferred.
Any related professional certifications or qualifications per federal or state regulations.
Working knowledge of social services laws and regulations.
Excellent communication and interpersonal skills.
Computer skills, including proficiency in Microsoft Office programs, medical records and clinical software programs.
