Job Description
Performs initial assessments to evaluate patient needs related to diabetes management.
Develops personalized educational plans for diabetes management and wellness.
Provides diabetes education for inpatients and outpatients through private sessions and group classes.
Monitors patient progress and provides ongoing support and education.
Collaborates with healthcare professionals to ensure comprehensive diabetes care.
Qualification Requirements
Associate's of Nursing (Required)
Bachelor's of Nursing (Required)
Master's
Basic Life Support - CPR Cert (BLS) – RQI Resuscitation Quality Improvement
Certified Diabetes Care and Education Specialist (CDCES) – EV Accredited Issuing Body
Licensed Dietitian/Nutritionist (LDN) – EV Accredited Issuing Body
Licensed Dietitian (LD) – EV Accredited Issuing Body
Registered Dietitian Nutritionist (RDN) – EV Accredited Issuing Body
Registered Nurse (RN) – EV Accredited Issuing Body
Pay Range $28.68 - $53.35
