Assists in arranging for medical care of the client at home and additional services needed for effective care of the client. The position reports to the Area Manager . SCHEDULE This is an in‑office position, Monday – Friday 8am‑5pm. BENEFITS Paid Time Off Recognition Program Employee Referral Program Tuition Reimbursement MAJOR RESPONSIBILITIES Reflects the company’s mission and values in developing and maintaining professional working relationship with others. Receives and coordinates all client referrals, which may include notifications of insurance providers for prior authorizations and services covered. Assists prospective clients and/or referral source(s) in identifying appropriate services to meet the client’s home care needs. Provides referral source with Company scope of service, coverage area, etc. as requested. Ensures quality relations and maintains a professional, high quality image to referral source. Mails correspondence and brochures to client/referral source when requested. Evaluates referral for clinical appropriateness of requested services. Verifies the clinical encounter note supports the primary diagnosis and meets payer guidelines when appropriate. Obtains any necessary verbal orders prior to initiation of services. Gathers client clinical and demographic information at time of referral/inquiry. Enters referral information into Company Electronic Medical Record. Communicates referral information to Clinical Supervisor/designee. Assists with assigning referral to clinician for assessment/start of care, as requested. Consults with office management and/or clinical staff, as needed. Maintains knowledge/education to remain current, efficient and productive as a home care resource to referral sources. Maintains confidentiality of all information pertaining to clients, families and employees. Maintains positive and effective communication with payers, clients, referral sources and employees. Demonstrates self‑direction to prioritize and accomplish job responsibilities. Using discretion and independent judgment in handling patient or physician complaints received, documents and forwards to appropriate administrative employees. Performs other related duties and responsibilities as assigned by the Area Manager. PHYSICAL/ENVIRONMENTAL DEMANDS Light to moderate - primarily desk work with frequent phone use; some bending, stretching. Ability to work long hours and to tolerate some variability in hours. Able to operate simple and complex office equipment Also may be necessary to drive a car to meetings and branch offices. QUALIFICATIONS Currently licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the State of Minnesota; home health experience preferred. The license must not have been revoked, suspended and without limitations or restrictions. Minimum of three years health care experience; intake and home health experience preferred. Knowledge of Medicare, private insurance and Minnesota Medical Assistance regulations. Ability to promote and maintain a positive attitude and encourage others to do the same. Able to handle stressful people and sensitive situations. Excellent written and oral communication skills, which includes effortless knowledge of English language. Good organizational skills and the ability to work independently with minimal supervision. Demonstrates ability to make appropriate clinical judgments as it relates to the referral process. Proficient with computers. Willing to learn new software. Knowledge of Word and Excel a plus. Current drivers license with proof of automobile insurance. Have U.S. Citizenship or evidence of valid Alien Work Permit. Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals/Entities. $51,500 - $76,500
per yearly. A candidate's salary is determined by various factors including, but not limited to, relevant work experience, skills, certifications, job level, supervisory status, and location.
