Job Summary : The Clinical Intake Coordinator acts as a liaison to referral sources, responding to company referrals and enabling home care services for patients.

Major Responsibilities

Reflect the company’s mission and values in professional relationships.

Receive and coordinate client referrals.

Assist prospective clients or referral sources in identifying appropriate services.

Provide referral source with company scope of service and coverage area.

Ensure quality relations and maintain a professional image.

Mail correspondence and brochures to clients or referral sources as requested.

Evaluate referral for clinical appropriateness.

Validate clinical encounter notes against payer guidelines.

Obtain necessary verbal orders prior to service initiation.

Gather client clinical and demographic information at the time of referral/inquiry.

Enter referral information into the company electronic medical record.

Communicate referral information to clinical supervisor or designee.

Assist with assigning referrals to clinicians for assessment or start of care.

Consult with office management or clinical staff as needed.

Maintain knowledge and education to remain current, efficient, and productive as a home‑care resource.

Maintain confidentiality of all client and employee information.

Maintain positive and effective communication with payers, clients, referral sources and employees.

Demonstrate self‑direction to prioritize and accomplish job responsibilities.

Use discretion and independent judgement in handling patient or physician complaints.

Perform other related duties as assigned by the Area Manager.

Physical/Environmental Demands Light to moderate—primarily desk work with frequent phone use; some bending and stretching. Ability to work long hours and handle variable hours. Operate office equipment and drive to meetings or branch offices as needed.

Qualifications

Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Minnesota (active, unrestricted).

Minimum three years of health‑care experience, preferably intake or home‑health.

Knowledge of Medicare, private insurance and Minnesota Medical Assistance regulations.

Positive attitude and ability to handle stressful situations.

Excellent written and oral communication skills, fluent in English.

Strong organizational skills and ability to work independently with minimal supervision.

Ability to make appropriate clinical judgments in the referral process.

Proficient with computers; knowledge of Word and Excel a plus.

Valid driver’s license with proof of automobile insurance.

U.S. citizenship or valid Alien Work Permit.

Successful completion of background investigations and screenings, including Minnesota Department of Health and the Federal Office of the Inspector General’s Excluded Individuals/Entities list.

Adara Home Health, Inc. is committed to fair and equitable compensation practices. The base salary range for this position is

$51,500 – $76,500

per year, based on experience, skills, certifications, and location.
