Care Manager LPN LVN - Case and Disease Government Program Providence is looking for a Care Manager LPN/LVN to coordinate care for Providence Health Plans members. Candidates residing in Oregon are encouraged to apply. The position is remote.

Description Provide care coordination services to PHP members, including disease management programs, education, motivation, and empowerment; case management including triage and referral, transition of care planning, end of life care planning, and support to advocate for members in achieving optimal health, access to care, and appropriate resource utilization. Services are offered to members and families with acute/complex needs, chronic conditions at risk for poor outcomes, and terminal/end‑of‑life patients.

Responsibilities Deliver the services described above, educate and motivate patients, advocate for their needs, coordinate transitions, evaluate health status, and support optimal health outcomes.

Required Qualifications Certificate/Diploma from a state-approved practical nursing program (major Nursing) Associate’s Degree in Healthcare or a related field Upon hire: Oregon Licensed Practical Nurse Upon request: Additional state licensure as assigned Upon request: Driving may be necessary; must provide proof of driver’s license and auto insurance 5 years of clinical nursing experience 2 years of experience working with physicians in collaboration and management of patient care

Preferred Qualifications Bachelor’s Degree in Health Education or a related field Experience in Care Management and/or Care Navigation in a healthcare setting Experience with Health Insurance, CMS, OHA, and other governing healthcare entities

Salary Range (Oregon) Non‑Portland Service Area: Minimum $32.92, Maximum $51.11 per hour Portland Service Area: Minimum $32.32, Maximum $54.82 per hour
