Care Manager LPN/LVN - Case and Disease Government Program (Remote)

Job Overview The purpose of this position is to provide care coordination services to Providence Health Plans (PHP) members, including disease management and case management. Services are offered to members and their families who have acute and complex health care needs, chronic conditions at risk for poor outcomes, or who are terminal and nearing end of life.
Responsibilities
Provide disease management programs: educate, motivate, and empower members to manage their disease.
Perform case management tasks: triage and referral, transition of care planning, end‑of‑life care planning.
Advocate and support members in achieving optimal health and accessing appropriate resources.
Collaborate with physicians and other health care professionals to coordinate patient care.
Required Qualifications
Certificate or Diploma from a state‑approved practical nursing program (Major in Nursing).
Associate’s Degree in Healthcare or a related field.
Oregon Licensed Practical Nurse upon hire.
Additional state licensure as assigned upon request.
5 years of clinical nursing experience.
2 years of experience working with physicians in the collaboration and management of patient care.
Driving may be necessary; proof of driver’s license and auto insurance upon request.
Preferred Qualifications
Bachelor’s Degree in Health Education or a related field.
Experience in Care Management or Care Navigation in a health care setting.
Experience with Health Insurance, CMS, OHA, and other governing health care entities.
Salary Range by Location Oregon (Non‑Portland Service Area): Min: $32.92, Max: $51.11 Oregon (Portland Service Area): Min: $32.32, Max: $54.82
