Attends work on a regular basis and may be required to work a specific shift schedule or rotating schedule, extended shift and/or overtime in accordance with agency leave policy. May serve in an on-call status as required. Performs routine monitoring of homes and grounds, addresses unusual occurrences, emergencies, and incidents immediately upon discovery; documents and reports findings. Serves as contact person for abuse/neglect allegations and serious incidents; responds immediately to ensure safety and health of individuals; liaises with the HHSC – Provider Investigations division. Acts as first responding professional staff on the scene of a critical incident, providing direction and guidance to staff and de-escalating the incident when possible. Coordinates and/or assists with unusual incidents including preliminary investigations to ensure compliance with rules and standards; participates as a member of the Incident Management Team with recommendations for issue resolution. Conducts special audits and monitoring to ensure ongoing service delivery and the health and safety of individuals served; follows up on issues and implementation of plans of correction. Provides input into Quality Assurance/Quality Improvement (QAQI) to improve facility operations and services. Instructs, trains, and provides in-service to facility staff on approved policies, procedures, and guidelines. Investigates and analyzes trends; assists in resolving technical problems and recommending activities to improve programs and operations. Other duties as assigned, including participation in disaster response/COOP activation; may require alternate shift pattern or location. Knowledge, Skills And Abilities (KSAs)

Knowledge of ICF/IID standards (or ability to gain knowledge). Knowledge of the facility Operating Procedures Manual (OPM). Knowledge of PNMP, PBSP, Ukeru and Safe Use of Restraints (SUR) techniques (or ability to gain knowledge). Excellent oral and written communication; ability to work harmoniously with coworkers. Good judgment in dealing with unusual incidents; ability to train staff in job-related subjects. Ability to perform SUR and Ukeru once trained; physical ability to perform direct contact duties and to complete regular rounds over a wide area. Qualifications

Initial Screening Criteria: Graduation from an accredited four-year college or university; and two (2) years of experience (IDD, emergencies/incidents, audits/monitoring, investigations of abuse/neglect/exploitation); supervisory or project supervision experience preferred; experience with unusual incidents in an ICF-IID or residential setting is preferred. OR high school diploma or equivalent; and six (6) years of relevant experience including the above items; supervisory or project supervision and unusual incidents experience preferred. Additional Information

Applicants must pass fingerprint background check, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, and LEIE). Males 18-25 must be registered with Selective Service. All State Supported Living Center Employees are subject to Random Drug Testing. Flexibility in work hours; may require overtime and/or weekends/holidays. All applicants must be at least 18 years old to be considered for employment at a state-operated facility.
