Position Summary
Oversee transitional housing opportunities for individuals diagnosed with a severe and persistent mental illness
Review clinical information, determine level of care, authorize treatment
Work on a team to actively coordinate ongoing client care and discharge planning to transition to less restrictive settings
Weekly case staffing with providers and clients
Daily collaboration in a fast‑paced team‑centered environment
What We Value In Our Next Triage Clinician
Bachelor’s or master’s degree in counseling, social work, or other mental health related field; and current Ohio licensure. LSW, LISW, LISW‑S, LPC, LPCC, and LPCC‑S are all desirable
Clinical assessment skills, understanding diagnostic criteria and symptom presentations outlined in the DSM‑5
Working knowledge of the Hamilton County behavioral health system of care preferred
A strong alignment with trauma‑informed, person‑centered care
