Position Summary The primary function of this role is to return patients to optimal physical function and facilitate the achievement of patient, physician, and therapy goals within a SNF setting. Instruct patients to maintain achieved levels of function after discharge through education, independent exercise programs, and appropriate activity modification.
Education & Qualifications
Bachelor’s degree or greater in Physical Therapy from an accredited college or university.
Active, unencumbered license granted from state of employment.
Basic computer skills and ability to navigate service delivery software and the electronic health record (EHR).
Shift & Wages
Full‑time opportunities available.
Shift: Varies.
Sign‑on Bonus: $5,000.
