Position Summary The primary function is to return patients to optimal physical function and facilitate achievement of patient, physician, and therapy goals within a skilled nursing facility (SNF). Instruct patients on maintenance of function after discharge through education, independent exercise programs, and appropriate activity modification.
Education and Qualifications
Bachelor’s degree or higher in Physical Therapy from an accredited institution.
Active, unencumbered state license.
Basic computer skills and ability to navigate service delivery and electronic health record software.
Shift & Wages
Full‑time opportunities available.
Shift schedule varies.
